THE BRIDGE OUTSIDE CASE MANAGEMENT SERVICES

Bridge provides intensive case management, in a one-on-one manner, for clients not residing at Bridge facilities.

Under this program, Bridge staff will engage with this client in daily case management meetings either at a Bridge facility or at the client’s home. Additionally, Bridge staff will provide medication management, transportation and treatment team liaison services, as well as being available 24 hours a day.

Bridge staff will monitor the client closely as he becomes more active in his treatment program and will communicate regularly with his treatment team.
